Uganda’s health records remain fragmented: some facilities use different electronic medical record systems, others use paper records, and some don't even share any records with patients. Patients also seek care from a variety of providers, ranging from drug shops, herbalists, clinics, pharmacies, and hospitals, leaving no complete record. We are working on a Digital Patient Book: an online platform where each patient can build one continuous medical history. Wherever they seek care, authorized clinicians can add visits, diagnoses, tests, treatments, and procedures. Patients can also upload records themselves, giving them a portable health story they can securely access and selectively share across clinics, hospitals, laboratories, pharmacies, and other providers.
